How do I keep the same parents from doing everything?

Limits do most of the work. Ask for one item per family, spread jobs across sections, and let unclaimed rows show plainly so you can ask the quieter half of the class directly.

Do I need permission from the teacher first?

Check with the teacher on party dates, food rules, and how many adults can be in the room. Then build the sheet around what they said and put those rules in the sheet title.
